Appointment requests
If you request an appointment using the form on this website, the following details are stored: first name, last name, date of birth, address, email address, telephone number, requested appointment and appointment type, together with any voluntary information about referral and your reason for the appointment. The time of the request and your IP address are also stored to prevent misuse.
Date of birth and address are required for the invoice you can submit to your health insurer.
If you provide information about symptoms or previous conditions in the free-text field, this constitutes health data under Art. 9 GDPR. This special category of personal data is processed on the basis of your explicit consent (Art. 9(2)(a) GDPR) and for the purposes of health care (Art. 9(2)(h) GDPR). Providing it is voluntary and not required to request an appointment.
The data is used solely to process your request and to manage appointments. It is not passed on to third parties. Access is restricted to the practice and subject to medical confidentiality.
Retention: Appointment requests are deleted automatically [24] months after the appointment. Statutory retention periods for medical records under § 51 Ärztegesetz (at least ten years) remain unaffected.

If you contact me by email or telephone, your details are stored in order to deal with your enquiry. Please note that unencrypted email is not considered a secure channel under data protection law. For health information I recommend the telephone or a personal consultation.
Maps
A Google Maps map is embedded to show the location of the practice (Google Ireland Limited, Gordon House, Barrow Street, Dublin 4, Ireland). When the map loads, your IP address is transmitted to Google and possibly to the United States. The legal basis is our legitimate interest in making the practice easy to find (Art. 6(1)(f) GDPR). Fonts
This website uses fonts loaded from Google Fonts. This establishes a connection to Google servers and transmits your IP address. The legal basis is our legitimate interest in a consistent presentation (Art. 6(1)(f) GDPR).

This website does not use cookies for analytics or advertising. Technically necessary cookies may be set by WordPress, for example when logging in to the administration area.
